Dear Sir/Madam

I recently had the misfortune of falling on the pavement near my home and hurt my knees.

The pavement in question is on Park Road North Houghton Regis. As you depart from Moore Crescent LU5 and turn right it is on the pavement on the same side as Moore Crescent. The pavement has a large log on it and is very muddy due to a car mounting the pavement a couple of weeks ago. Please find attached a photo of the pavement. I hope you can address the condition of the pavement as it is a health and safety issue.

Look forward to hearing from you with feedback and a resolution to how you will proceed to improve the condition of the pavement.
